Then went to presentation by Geraint Lewis regarding Croydon’s Virtual Ward. They’re using the Combined Predictive Model for case finding (as we will shortly) and what they do looks of great interest. Wandsworth apparently starting soon as well.

15 May 2008 by AdrianThis site has been started following the first meeting of the Lambeth Intermediate Care Project Board.
The intention is that all relevant information that can and should be in the public domain can be posted here, and available for anyone to read, and importantly to comment upon.
Hopefully a number of people will volunteer or be volunteered to maintain this. Any offers, contact adrian.mclachlan@gmail.com
I will try and knock together some background reading and links, and would welcome help with relevant and current policy documents etc.
